DOT Physicals require a separate appointment from a Drug & Alcohol Test

Notice: Starting January 8, 2026, a 2.7% surcharge will apply to all credit card transactions.
To avoid this fee, we welcome payment by cash or check.

What to Bring:

  • Valid Driver’s License - for both Drug Collection and/or BAT

  • Chain of Custody - for Drug Collection (if you don’t have one, we have house copies that can be used for an additional fee)
